We accept all major credit cards as forms of payment. All payments are due at the start of your scheduled appointment.
Cancellations & Rescheduling Policy
Your time and effort are valuable to us – and we understand that extenuating circumstances may take place. Please call us directly if you need to reschedule your appointment. If you would like to cancel your visit, please call us within 24-hours of your scheduled time.
Insurance information:
We are currently a cash-based practice, meaning we are out of network with insurance providers. As an out-of-network provider, the goal of Livality Integrated Health is to provide more individualized care, better accessibility, and longer visits in an effort to provide tailored treatment to you. Many insurance plans offer out-of-network reimbursement for provider visits, at times providing up to 80% reimbursement of costs paid. If you would like to submit a claim to your insurance, we can provide a superbill for your convenience. You may also call and ask your insurance ahead of time if mental healthcare is a covered out of network service. They may ask for CPT codes, which are noted as below:
- Initial Psychiatric evaluation: 90792
- Psychiatric medication management follow-ups: 99213, or 99214+ 90833 for brief supportive therapy.
*Insurance coverage for select carriers will be available in the near future.
The goal of Livality Integrated Health is to provide individualized & patient-centered care, better accessibility, and longer visits in an effort to provide tailored treatment to you. We are pleased to accept select insurance plans, with more continually being added over the next few months. We are also able to accept Cigna, Optum, and Aetna through the Alma portal. We currently do not accept Medicaid or Medicare, but sliding scales fees are available for those that may qualify.
Assistance is offered for out-of-network (OON) plans. Many insurance plans offer out-of-network reimbursement for provider visits, at times providing up to 80% reimbursement of costs paid. If you would like to submit a claim to your insurance, we can provide a superbill for your convenience. It is recommended that you call and ask your insurance ahead of time if mental healthcare is a covered out of network service. To be prepared, have the usual CPT codes we utilize, which are noted as below:
- Initial Psychiatric evaluation: 90792
- Psychiatric medication management follow-ups: 99213, or 99214+ 90833 for brief supportive therapy.
Mental Health self-pay pricing:
- Initial Evaluation $250
- Medication management follow-up: $100-150
- Psychiatric Consulting- contact clinic
Wellness Visit self-pay pricing:
Weight loss treatment, IV hydration, Primary care needs, labs, etc
*Please note, these are out-of network/non-covered services through insurance and will be cash-pay only. If labs have not been obtained in the 3-6 months prior to appt; they will need to be updated. Insurance coverage may cover those costs.
- Monthly or one-time costs will vary depending on the service selected.
- For GLP-1 medication (compounded semaglutide or tirzepatide), costs vary depending on dosage of medication.
